Second, to process your claim for a bereavement benefit the FBA will require the following documentation:
- Certificate of Death issued from the respective state or agency. Please note that a copy can NOT be accepted. This must be an original document affixed with the official seal of the issuing agency.
- A copy of the designated beneficiary’s identification. This may be a state issued driver’s license or ID card.
- A copy of the deceased member’s identification. This may be a state issued driver’s license or ID card.
Optional: Any Certificate of Benefits previously issued by the Firemen’s Benevolent Association.

We know that during a confusing time like this, even simple decisions can seem huge. And no matter how well you may have prepared, you may feel that you are forgetting something important. So, we have provided you with some information that may be of help.
First, please notify the FBA office of your loss. This may be done via the NOTIFICATION form provided or by contacting the FBA office directly at 813-740-0872 where a representative will assist you.
