To better meet your needs and speed the processing of your claim, we want to make sure you understand our claims process and the documentation required. First, please notify the FBA office of your loss to verify the member’s beneficiary designation information. This may be done via the NOTIFICATION form provided or by contacting the FBA office directly at 813-740-0872 where a representative will assist you.

Second, to process your claim for a bereavement benefit the FBA will require the following documentation:

  • Certificate of Death issued from the respective state or agency. Please note that a copy can NOT be accepted. This must be an original document affixed with the official seal of the issuing agency.
  • A copy of the designated beneficiary’s identification. This may be a state issued driver’s license or ID card.
  • A copy of the deceased member’s identification. This may be a state issued driver’s license or ID card.

Optional:    Any Certificate of Benefits previously issued by the Firemen’s Benevolent Association.
